Whats the worst mistake/goof you have seen in a Cartoon Cartoon?

Just wonderin, what do you think is the worst goof/mistake you have seen?

There have been some "slips" on ....Robot Jones, but those aren't bad, they make it even funnier. Like a poster in the school hall that says "PMS" on it or a bowl of fruit with some of the fruits
arranged to.....I won't get into that.
Those are goofs, but they aren't bad. They're funny! Can you believe they got away with that?
