I would create the best DVD case spindle ever--something that would never bite your finger off or hold so well that you have to bend the disc in half to get it out. Then I would release the design into public domain.
The problem is that there are many, many case manufacturers out there now and they all have to use slightly different spindles because they all have patents on their own and they can't rip each other off. There needs to be a unifying solution that would end crummy spindles once and for all.
We need a public domain spindle design!

BTW, for those that care, here's a visual list of many types of these DVD cases: http://www.math.ttu.edu/~kesinger/dvd/ (for the record, my least favorites are Scanavo and Protect-o-Case Type B)
And I fully agree. You never really know what type of case you're going to get until you open it up, and you need a different technique to get them out without cracking the disc for each one (Scanavo being one of the toughest to get out). Bah humbug.
Also, whoever invented cases with the two latches that you need to unlatch before you open the case needs to be fired.

What I find interesting is that Alpha, Amaray, and THINPaks are all by the same company, Nexpak. I guess it's their fault for making so many designs.

It's always nice to open up a new DVD and have it be in an Amaray. (Two- or three-pronged, it doesn't matter!)

M-lock FTW.
I'm glad Bandai seems to be using them for a lot of recent releases, as opposed to those smelly push-click things they've been using (similar to Unknown 9 on that site).
